Physical activity is known to reduce cardiovascular risk, but it is unclear which type of physical activity or modality (aerobic vs. resistance) has greater protective effect against future cardiovascular events. We evaluated the effect of aerobic and resistance exercise on the risk of future cardiovascular events in a population-based cohort of Swedish adults. A total of 1755 men and women participated in a health survey in 1994 and were followed up until 31 December 2000 for incident cardiovascular events. Questionnaires on self-reported exercise habits were mailed in 1994. A total of 1290 subjects (72% women) with valid data on exercise habits and follow-up were included in the analyses. We calculated hazard ratios (HR) with 95% confidence intervals (95% CI) using Cox regression analyses for associations between exercise habits and future cardiovascular events. During a mean follow-up of 8.7 years, 396 subjects had a cardiovascular event. Subjects who were physically active in 1994 had a lower risk of future cardiovascular events compared with those who were inactive (HR=0.62, 95% CI 0.45-0.85). Aerobic exercise was associated with a lower risk of cardiovascular events than resistance exercise, when comparing highest to lowest exercise intensity in aerobic exercise. However, when comparing highest to lowest exercise intensity in resistance exercise, there was no difference in risk. There was no difference in risk between aerobic and resistance exercise when comparing high to low exercise intensity. Exercise habits in 1994 were positively associated with a lower risk of future cardiovascular events in a population-based cohort of Swedish men and women.
